Form I-90 is used by U.S. lawful permanent residents and conditional permanent residents to replace their green cards. Form I-90 is officially called Application to Replace Permanent Resident Card. You need to apply this application form when your green card expire or will expire within 3 months (90 days). Additionally, you can use Form I-90 to replace a green card that was damaged, or when your personal data was changed.

Conditional permanent residents cannot apply Form I-90 to replace their green card if the card has reached the expiration date or will expire in 3 months. Instead, they must file a different form – either Form I-751 or Form I-829 – to the U.S. Citizenship and Immigration Services agency.
