You should complete this form if you are a United States citizen, a U.S. national or a lawful permanent resident age 18 or older who is acting as a sponsor or petitioner for someone who is seeking an immigrant visa overseas or seeking a Green Card through adjustment of status while in the United States.
Use this form to demonstrate that you are financially capable of providing financial support for the person you are sponsoring and the rest of your household by meeting certain criteria. By signing Form I-864, you are entering into a contract with the U.S. government and promising to support the person you are sponsoring if necessary.
Immigration law mandates that the following immigrants submit Form I-864 after it has been completed by the petitioner/sponsor to obtain an immigrant visa overseas or to adjust status to that of a lawful permanent resident in the United States:
- All immediate relatives of U.S. citizens (spouses, unmarried children under 21 years of age, and parents of U.S. citizens 21 years of age and older);
- All family-based preference immigrants (unmarried sons and daughters of U.S. citizens, spouses and unmarried sons and daughters of lawful permanent residents, married sons and daughters of U.S. citizens, and brothers and sisters of U.S. citizens 21 years of age and older); and
- Employment-based preference immigrants, but only in cases where a U.S. citizen, lawful permanent resident, or U.S. national relative who filed the immigrant visa petition or has a significant ownership interest (five percent or more) in the business that filed the petition.

To complete this form, answer all questions fully and accurately by typing or printing neatly in black ink.
If you need extra space to complete any item within this affidavit, use the space provided in Part 11, Additional Information. You may also attach a separate sheet of paper as long as you type or print your name and Alien Registration Number (A-Number) (if any) at the top of each sheet. You must also indicate the page number, part number, and item number to which your answer refers. Do not forget to sign and date each sheet.
If a question does not apply to you, put “N/A,” in the designated space unless otherwise directed. If your answer to a question which requires a numeric response is zero or none put “None,” in the applicable space unless otherwise directed.

Do not forget to sign the form. Once you have done so, give the completed form and supporting documents to the immigrant you are sponsoring to file with their Form OF-230 or Form I-485.
