You can use Form I-90, Application to Replace Permanent Resident Card, if you are a lawful permanent resident or you are a conditional permanent resident. However, conditional permanent residents cannot use this form to remove the conditions on their Green Cards, to replace an expired card, or to replace a card that will expire in 90 days for any reason.
Use this form to apply for a new Permanent Resident card if: your card has been lost, stolen, or destroyed; it was issued but you never got it; it has been mutilated; your current card has incorrect data because of DHS error; or your name or other information has been legally changed.
You can also use Form I-90, Application to Replace Permanent Resident Card if: your card will expire within six months or has already expired; you have reached your 14th birthday, and your existing card will expire after your 16th birthday; you have reached your14th birthday and your existing card will expire before your 16th birthday; you are a permanent resident who is taking up commuter status; you are a commuter who is taking up residence in the U.S.; you have been automatically converted to permanent residence status; or you have a prior edition of the card, or are applying to replace your card for another reason.

Complete the Application to Replace Permanent Resident Card by typing or writing neatly in black ink. Use the space provided in Part 8, Additional Information, if you need more room to answer any question on the form. You may also attach additional sheets of paper as long as you type or print your name and Alien Registration Number (A-Number) (if any) at the top of each sheet; indicate the Page Number, Part Number, and Item Number to which your answer refers; and sign and date each sheet. Be sure to answer all questions truthfully and completely. If a question does not apply to you, put “N/A” in the space for the answer unless instructed differently. Put “None” in response to any question requiring a numeric answer that does not apply to you unless instructed differently. Do not forget to sign the application; USCIS will not accept an unsigned form.
