Part 5 of Form I-751, Petition to Remove Conditions on Residence, is intended for providing information on all of your children.

In Form I-751 Part 5 you must provide the following information on each of your children separately: full name, date of birth, A-Number (if any), whether the child is living with you, whether the child is applying with you, physical address of the child.

If you have a dependent child who acquired conditional resident status on the same day as you or within 90 days thereafter, then in order to request that the conditions on his/her status be removed as well, you should tick the box “yes” in the question “Is this child applying with you”. And if you have dependent child/children who did not acquire conditional resident status on the same day as you or within 90 days thereafter, or if the conditional resident parent is deceased, then those dependent children must each file Form I-751 separately.

Part 5 contains fields for 5 children only. If you need extra space to complete this section use the space provided in Part 11 “Additional Information”.

Also please note that each conditional resident dependent listed under Part 5 of Form I-751 is required to submit an additional biometric services fee of $85 (including dependents residing overseas pursuant to military or government orders, regardless of age). Meanwhile, the filing fee will be charged in the amount of $595, no matter how many individuals are included into one Form I-751.
