Part 9 of Form I-131 is called “Information About Person Who Prepared This Application, If Other Than the Applicant”. This section needs to be filled out only if someone else prepared your application. Otherwise, just leave it blank.
When the preparer is an attorney or an authorized representative, he/she must additionally submit Form G-28, Notice of Entry of Appearance as Attorney or Accredited Representative, along with your Form I-131, Application for Travel Document.
In Form I-131 part 9 it is required to provide the following information concerning the preparer: preparer’s full name, his/her business or organization name, mailing address, phone number, e-mail address, preparer’s signature, and date of signature.
By putting the signature in Part 9 the preparer agrees to the following declaration stated in the form: “I declare that I prepared this benefit request at the request of the applicant, that it is based on all the information of which I have knowledge, and that the information is true to the best of my knowledge.”
Please note that in case you have any additional information and require more space, you should use a separate sheet of paper, and include your name and A-Number on the top of each sheet.
