Just as with all the other USCIS forms, your Form N-565 must be properly signed and filed.
First of all, use the most current form version downloaded from the official USCIS website.
Fill out Form N-565 accurately, complete all sections of the form. Answer all questions fully. If a question does not apply to you type or print in black ink “N/A,” unless otherwise directed. If a question requires a numeric response and your answer is zero or none simply type or print “None,” unless otherwise directed.
If you need extra space to complete any item within the application, attach a separate sheet of paper; type or print your name and A-Number (if any) at the top of each sheet; indicate the page number, part number, and item number to which your answer refers. Sign and date each sheet. You may also number the pages and include the total amount of pages being attached.
Do not forget to sign your form, otherwise it will be rejected. Also, note that USCIS will not accept a stamped or typewritten name in place of a signature.
If you make an error, start over with a clean form.
And of course do not forget to attach copies of the supporting documentation written in English or accompanied by a complete English translation.
