Depending on your circumstances and the immigrant classification you have chosen, you can file Form I-485 with USCIS as a principal immigrant or a derivative applicant. For more information, see the Instructions for Form I-485 on the USCIS website.
Fill the Form I-485
To complete this form, type or print the answers in black ink. Be sure to write neatly and use the space provided in Part 14, Additional Information if you need extra room. You may also use extra sheets of paper as long as you type or print your name and Alien Registration Number (A-Number) (if any) at the top of each sheet. You must also indicate the page number, part number, and item number. Do not forget to sign and date each sheet.
You must answer all questions fully and accurately. For questions that do not apply to you, put “N/A” in the applicable space unless you are instructed differently. For any question that warrants a numeric response and your answer is zero, or none, put “None” in the applicable space unless otherwise directed. You must sign and date the form; USCIS does not accept unsigned forms.

Document Checklist Form I-485
USCIS asks for different types of evidence, or supporting documents from applicants in different immigration categories. In general, you’ll be asked to provide two passport-style photos that meet specific requirements; copies of a government-issued Photo ID; a photocopy of your birth certificate; information from your passport (copies of certain pages) and/or arrival/departure records; and medical records and/or the results of your medical exam. You must also provide the documents for your specific immigration category.
USCIS may reject or deny your application if you do not provide all requested material along with your application. Failure to provide it may also result in processing delays not only for your Form I-485 but associated applications as well.
If you can’t primary evidence (such as a birth or marriage certificate), USCIS may accept secondary documents such as church or school as long as you can explain why the primary evidence is unavailable. If you are unable to submit secondary evidence, you may provide two or more sworn statements from people who are not associated with the immigration benefit you are seeking, but have direct personal knowledge of the event and circumstances. You must also explain why you can’t provide the primary and secondary supporting documents.
Most (but not all) applicants must also send payment for the filing fee and biometrics services fee (if applicable) along with Form I-485.

Acceptable forms of payment include money orders, personal checks money or money orders. Credit card payments are also acceptable if your form is filed at a USCIS Lockbox facility as long as you use Form G-1450, Authorization for Credit Card Transactions. If you pay by check, you must make your check payable to the U.S. Department of Homeland Security.
