On Form I-90, Application to Replace Permanent Resident Card, you will see the term “adjustment of status” mentioned several times in the Part 3. The adjustment of status is one of the procedures that you can use to obtain a green card in the United States.
If you were in the U.S. at the time when you applied for a green card, it means you applied the Form I-485 to U.S. Citizenship and Immigration Services (USCIS). Therefore, to correctly fill out Form I-90, write the city and state of the USCIS office into the item 1 in Part 3 to specify the location where you applied for an adjustment of status. The item 2 in Part 3 also requires you to write the city and state of the USCIS office that granted you the adjustment of status.
